Shared Care

Shared Haemodialysis Care means working with patients in partnership to engage them in their own treatment. It encourages patients to take an interest in helping themselves- promoting a more positive feeling towards their dialysis.

Shared Haemodialysis Care puts patients back in control.

Opportunity

All patients can be offered the chance to learn more and to participate in their dialysis care

No assumptions are made about their ability or desire to take part

Choice

Patients can participate at a level of their choosing and follow their own personal path to suit their needs making them feel safe and comfortable to engage when they are ready

Motivation

Attention is paid to what interests and motivates patients so that learning is meaningful and fulfilling

There are many ways in which patients can engage in their own dialysis care from learning about their condition and how haemodialysis works to participating in any of the tasks involved such as:

‚ÄčSetting up their
own trolley for
the treatment

Many patients tell us that they enjoy being involved and that it helps them to understand and feel in control of their condition:

One of the down sides of being on dialysis is that you feel you have lost control of your life. Taking part in Shared Care has given me back some control, a feeling of self- esteem and achievement and a better understanding of my care.

ELAINE, SHARED CARE PATIENT

The course has really motivated me. I want to help my patients become far more involved and I can see that patients participating can help them cope and feel much more positive about coming for dialysis.
